You are a PA working in A&E and you are seeing a 70-year-old male who has presented with shortness of breath and a fever. This patient does not appear oriented to person, place or time and his family who are with him report that this is not like him. You do some blood tests and observations. His BP is 85/60, HR 133, O2 saturations 95% on room air, RR 31 and his temperature is 38.4. His blood tests report that he has raised inflammatory markers and a urea of 6mmol/litre. What is the most appropriate next step in this patient’s management?
