This patient has been experiencing episodes of recurrent otitis media over the past few months which have been treated successfully with antibiotics. On examination you note that this patient has intense pain behind the ear, discharge coming from the ear and a bulging tympanic membrane on otoscopic examination. What is the most likely diagnosis?
