You see a 60 year old male in A&E who complains of sudden onset abdominal pain. He cannot sit still and has vomited twice in the last two hours. He tells you that he has experienced four bouts of diarrhoea today and a fever. On examination he looks bloated and does not let you palpate his abdomen due to the pain.
